About This Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Yale New Haven Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities
  1. Assist with Patient Care: Under the supervision of the Nurse Manager and Registered Nurse, you will perform various tasks to support patient care, including direct and indirect patient care activities.
  2. Obtain and Record Vital Signs: You will obtain and record vital signs as ordered and delegated by the Registered Nurse.
  3. Perform Technical Duties: You will perform technical duties and selected treatments, procedures, and activities that contribute to safe and efficient delivery of patient care.
  4. Assist with Patient Safety: You will effectively assist in the care of patients with consideration for the safety of key populations.
  5. Communicate Effectively: You will effectively and professionally communicate and interact with patients, families, and staff.
  6. Participate in Multidisciplinary Team: You will collaborate with members of the multidisciplinary team, offering suggestions on addressing patient/family needs, taking into account cultural and/or individual diversity.
Requirements
  • Education: High school diploma or GED; and CNA or other educational equivalent.
  • Experience: Previous experience in a healthcare setting, preferably in adult psychiatric care.
  • Licensure: CT CNA certification, EMT, or student nurse from an accredited program.
  • Skills: Basic Cardiac Life Support (BCLS) training, ability to speak and write English proficiently, and successful completion of orientation.
Working Conditions

This role requires the ability to sit, use hands to finger, handle, or feel, reach with hands and arms, and talk or hear. You will also be required to stand and walk, and occasionally lift and/or move up to 25 pounds.
