Case Manager

Job Summary
Insight Global is seeking a highly skilled Case Manager to provide comprehensive support and services to veterans in need. The ideal candidate will possess strong interpersonal skills, a deep understanding of veterans' issues, and the ability to coordinate with multiple agencies and service providers.
Key Responsibilities
- Assess clients' needs, strengths, and barriers and develop effective housing stability plans.
- Provide case management services, including regular home visits, housing counseling, client-centered advocacy, referrals, and crisis intervention.
- Manage a caseload of veterans and/or veteran families, including traditional, Shallow Subsidy, and HUD-VASH cases.
- Effectively communicate the Priority Veteran program to prospective clients, landlords, and community members.
- Maintain consistent contact with local homeless coalitions, VA Hospital, shelters, and other relevant organizations.
- Follow up on all inquiries in a timely manner and accurately conduct eligibility determinations.
- Maintain accurate and timely case documents.
- Follow grant regulations and program policies.
- Participate in professional development training and weekly staff meetings.
Working Conditions
Services are provided in office, in the community, and in client homes. Some occasional after-hours/weekend work may be required.
Pay: $21.15/hr, Conversion Salary: $40,000 - $44,000/yr
