We are seeking a skilled Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will be responsible for assessing patients' needs and developing effective treatment plans in a supportive and positive environment.
Your primary focus will be on aiding patients in healing and maximizing their functional independence while ensuring patient confidentiality and care standards are met.
Responsibilities:- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
- Prepare for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting appropriate tests and measures.
- Formulate diagnoses, goals, prognoses, and plans of care based on evaluation results.
- Establish treatment plans, collaborate with interdisciplinary teams, and assess the need for modifications.
- Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements.
- Valid CPR license and experience/knowledge with EMR software (preferred).
- Excellent oral and written communication skills, strong clinical decision-making abilities, and a positive attitude.
- A competitive salary ranging from $85,000 to $115,000 per year, depending on location and experience.
- A sign-on bonus of up to $7,500.
