Job Description

As a Physical Therapist - Rehabilitation Specialist at JAG Physical Therapy, you will be responsible for assessing patients' needs and developing effective treatment plans to enhance their recovery and functional independence. Your duties will include:

  • Collecting and recording patient care data, following format and guidelines of the patient care service, regulatory agencies, and third-party payers
  • Conducting initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team
  • Administering tests and measures, synthesizing evaluation results to formulate diagnosis, goals, prognosis, and plan of care
  • Establishing treatment plans based on evaluation results, goals, and best practice, collaborating with interdisciplinary teams, patients, and patient support systems
  • Assessing and re-assessing need for modifications to plan of care and goals, identifying need for consultations and/or referring patients to other healthcare professionals or services
  • Initiating and completing discharge plans, providing education/training to patients and/or patient support systems, and overseeing support staff
Required Skills and Qualifications

To be successful in this role, you will need:

  • Bachelor's or Master's degree in physical therapy, Doctorate degree preferred
  • Licensed physical therapist in the state practicing in
  • Completed state-regulated CEU requirements in the appropriate state (NY, NJ)
  • Valid CPR license
  • Experience/knowledge with EMR software preferred
  • Excellent oral and written communication skills
  • Strong clinical decision-making skills
  • Excellent work ethic and dedication to patient success
  • Basic computer skills
  • Ability to work as a team member and individually with minimal supervision
