- Outreach and Engagement: Conduct outreach to shelters, streets, hospitals, and community settings to meet clients where they are at and provide services in a culturally responsive and trauma-informed manner.
- Documentation and Reporting: Ensure timely and accurate documentation of all services provided, maintaining applicable chart requirements and adhering to county, state, and federal regulations.
- Inter-Agency Collaboration: Participate in inter-agency collaboration to facilitate problem-solving and clinical care, ensuring seamless care coordination and linkage to community-based supports.
- On-Call Consultation: Provide on-call consultation during working hours to triage clinical emergencies and coordinate coverage for the team.
- Additional Duties: Perform additional duties as needed, including serving as a backup for the team and attending assigned trainings for professional, program, and agency development.
- Master's Degree: Master's degree in Social Sciences, Psychology, Counseling Psychology, or a related field, with current California State licensure or registration as an associate.
- Experience: Two years of experience providing mental health services to individuals with serious mental health challenges, substance use, and homelessness.
- Modalities: Experience with specific modalities, including harm reduction, contingency management, substance use disorder treatment, crisis intervention, de-escalation, and motivational interviewing.
- Outreach Experience: Experience conducting outreach in settings that include shelters, street encampments, SROs, hospitals, and other areas as assigned.
- Charting and Documentation: Demonstrated proficiency in charting and documentation under insurance payors, including Medi-Cal, Medi-Med, and special population funding.
- Technology Skills: Demonstrated proficiency in Microsoft applications, phone-based applications, and navigating EHR systems, including EPIC and CIRCE experience.
- Language Skills: Bi-lingual in threshold languages, including Spanish, Cantonese, Mandarin, Vietnamese, Russian, and Tagalog, is strongly preferred.
- Mental Health and Substance Use Disorders: Strong working knowledge of mental health and substance use disorders, including DSM diagnostic skills, clinical interviewing, assessment, and use of available medical records to inform assessment.
- Psycho-Education: Ability to provide psycho-education to clients and consultation to community partners to support the client in their recovery process.
- Triage and Crisis Intervention: Effective triaging skills to prioritize work based on risk and client need, crisis intervention skills, and ability to establish trusting, hopeful relationships with clients and community supports.
- Interdisciplinary Team Effectiveness: Strong knowledge of county behavioral health department and community resources for linkage support, interdisciplinary team effectiveness, and ability to provide, receive, and integrate performance, quality, and best practices feedback into care provision.
- Communication and Interpersonal Skills: Excellent communication (written and verbal) and interpersonal skills, adaptable, open to change, and able to tolerate ambiguity.
- Organizational Skills: Strong organizational skills, attention to detail, ability to prioritize tasks, and manage time effectively.
- Team Player: Demonstrated experience as a team player with a welcoming attitude, knowledge of County, State, and Federal requirements for documentation and reporting, including Medi-Cal/Medi-Med standards, CPT codes, etc.
