Job Overview

The Oncology Clinical Nurse Navigator utilizes specialized nursing expertise to provide tailored support to patients, families, and caregivers, helping them navigate the complexities of the healthcare system. Through the application of the nursing process, the Clinical Nurse Navigator delivers education and resources that empower informed decision-making and ensure timely access to high-quality health and psychosocial support throughout the cancer care continuum. Responsibilities include overseeing routine nursing functions, such as clinical care, telephone triage, symptom management, and patient education, while also monitoring and following up with patients. Collaboration with various healthcare professionals, including schedulers, medical assistants, infusion nurses, and physicians, is essential for the effective execution of daily clinical operations.

Key Responsibilities:

  • Manage nursing care for a designated group of patients within the hematology/oncology specialty, focusing on assessment, education, coordination, and documentation.
  • Serve as the primary contact for physicians, patients, and caregivers to facilitate care coordination.
  • Ensure a seamless transition for patients from diagnosis through active treatment to survivorship and end-of-life care.
  • Act as a liaison among multidisciplinary team members and various departments to integrate relevant patient information for a cohesive care experience.
  • Handle and triage patient calls, addressing specific needs promptly and notifying physicians when necessary.
  • Conduct follow-up communications with patients to ensure adherence to treatment plans and to assess any evolving clinical situations.
  • Collaborate with Financial Advocates to secure financial assistance for treatment options.
  • Work alongside the Prior Authorization team to facilitate necessary treatment approvals.
  • Provide psychosocial support and appropriate referrals for patients, families, and caregivers.
  • Develop standardized care plans in conjunction with physicians, including referrals and clinical trials.
  • Collaborate with the healthcare team to create and update patient care plans that are clear and actionable for patients and their families.
  • Design and deliver educational materials for patients.
  • Utilize technology (e.g., EMR, patient portals) to enhance communication between patients and healthcare providers.
  • Create and maintain documentation tools to track interactions with patients, families, and caregivers.
  • Exhibit leadership qualities by motivating, educating, and mentoring peers while managing patient care effectively.
  • Be prepared to travel and work at various locations as needed.
  • Perform additional duties as assigned.

Licenses & Certifications:

  • Current Registered Nurse license in the relevant state.
  • BLS Certification is required.
  • Oncology Nurse Certification is preferred at hire but must be obtained within two years of employment.
