Job Overview

The Oncology Clinical Nurse Navigator leverages their specialized nursing expertise to provide tailored support to patients, families, and caregivers, assisting them in navigating the complexities of the healthcare system. By employing the nursing process, the Clinical Nurse Navigator delivers education and resources that empower informed decision-making and ensure timely access to high-quality health and psychosocial services throughout the cancer care continuum. This role encompasses essential nursing responsibilities, including clinical care, telephone triage, symptom management, oversight of patients on oral chemotherapy, data collection, patient education, and ongoing monitoring and follow-up. The Clinical Nurse Navigator collaborates closely with schedulers, medical assistants, non-clinical navigators, infusion nurses, and physicians to effectively manage daily clinical operations.

Key Responsibilities:

  • Takes on nursing care responsibilities for a designated group of patients within hematology/oncology, focusing on assessment, education, coordination, and documentation.
  • Acts as the primary contact for physicians, patients, and caregivers to facilitate the coordination of care.
  • Ensures a seamless transition for patients from diagnosis and active treatment to survivorship, chronic cancer management, and end-of-life care.
  • Serves as a liaison among multidisciplinary team members and various departments/specialties to integrate vital patient information for a cohesive continuum of care.
  • Receives and triages patient telephone inquiries, responding appropriately to meet individual patient needs promptly. Notifies physicians as necessary for patients requiring medical intervention.
  • Conducts follow-up calls to patients as needed to ensure adherence to treatment protocols and to assess evolving clinical situations.
  • Collaborates with Financial Advocates to secure financial assistance for all treatment modalities.
  • Works in conjunction with the Prior Authorization team to facilitate necessary authorizations for treatments.
  • Provides psychosocial support and coordinates appropriate referrals for patients, families, and caregivers.
  • Develops standardized care plans with physicians, including referrals, bone marrow transplants, drug protocols, and clinical trials.
  • Partners with the multidisciplinary team to create and/or update the patient's care plan, ensuring it is clear and actionable for the patient, family, and caregivers.
  • Designs and delivers patient education materials.
  • Utilizes technology (e.g., EMR, My Chart) to enhance communication between patients and healthcare providers.
  • Develops and employs documentation tools to track interactions with patients, families, and caregivers.
  • Demonstrates leadership by motivating, educating, delegating, and mentoring peers and other healthcare team members while effectively managing patient care.
  • Must be willing to travel and work at various locations as required.
  • Performs other duties as assigned.

Qualifications:

Knowledge and Skills:

The following specialized knowledge and skills are essential for this position:

Education: Graduate of an accredited nursing program. A Bachelor of Science in Nursing (BSN) is strongly preferred, with equivalent combinations of nursing education and RN experience considered. Additional training in Nurse Navigation or Oncology Certification is preferred.

Experience: Minimum of 5 years in a relevant nursing role.

Licenses & Certifications:

  • Current Registered Nurse license or interim permit in the relevant state.
  • Basic Life Support (BLS) Certification is required.
  • Oncology Nurse Certification is preferred at hire but must be obtained within two years of employment.
