We are excited to offer an exceptional opportunity for a skilled Support Specialist to join our team, dedicated to providing top-notch support to individuals with disabilities.

About the Job:

As a Support Specialist, you will collaborate with our clients residing in Anoka, Hennepin, or Dakota County areas to develop and implement tailored support plans that address their unique needs. You will assist them in accessing relevant services, evaluating treatment options, creating care plans, coordinating care, and monitoring progress to ensure they receive the best possible services.

Key Perks:

A competitive salary range of $45,000-52,000 per annum, based on experience

Main Responsibilities:

  • Tailored Support Plans: Develop and implement customized plans for each individual, focusing on their strengths and challenges (Typical caseload: 40-45 individuals)
  • Service Coordination: Assist individuals in finding and accessing suitable services, creating service agreements, and maintaining accurate case records
  • Needs Assessment: Conduct thorough assessments to understand individual needs, preferences, and goals
  • Progress Evaluation: Regularly meet with individuals to evaluate progress, adjust support plans as needed, and identify potential providers to help them reach their objectives
  • Compliance and Reporting: Ensure adherence to Case Management Rule 185 and compliance with Minnesota Health Care Programs regulations regarding waiver services delivery

Requirements:

Meet ONE of the following criteria:

  • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
    • If holding a Social Work degree, be currently licensed as a Social Worker, as required by the Minnesota Board of Social Work.
  • OR Bachelor's degree in any other field with at least one year of experience working with individuals with mental illness or disability.
  • Good written and oral communication skills.
  • Excellent ability to work independently and in crisis situations.
  • Comfortable with high-volume paperwork.
  • Attention to detail and process-oriented.
  • Able to pass a DHS background check, valid driver's license, and clean driving record.
  • Reliable transportation.
