Hospice LPN

We are seeking an experienced Hospice Licensed Practical Nurse (LPN) to join our team at Liberty Hospice. As a Hospice LPN, you will provide direct patient care, coordinate services, and work collaboratively with our interdisciplinary team to deliver high-quality care to our patients and their families.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Coordinate hospice services with the Registered Nurse, Patient Care Coordinator, and other agency personnel to optimize patient care.
- Document patient information accurately and thoroughly, including progress notes, summary notes, and other relevant documentation.
- Attend and participate in Interdisciplinary Team (IDT) meetings to discuss patient care and develop plans of care.
- Accept and transcribe physician's orders as directed.
- Successful completion of nursing education from an approved school of nursing.
- Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
- Ability to independently organize work and provide effective health care services to hospice patients.
- Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
- Effective communication skills (oral and written).
- CPR certification.
