Job Summary

We're seeking a skilled LPN Long-term Care professional to join our team at our Burlington campus. As a member of our interdisciplinary team, you'll work closely with patients, caregivers, and other healthcare professionals to provide high-quality care and support.

Responsibilities
  • Provide basic personal care and support to patients with limited life expectancy
  • Administer medications and treatments as directed
  • Document patient care and maintain accurate records
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
  • Participate in weekend and weeknight coverage to ensure patient care continuity
Requirements

To be successful in this role, you'll need:

  • A current North Carolina LPN license
  • One year of LPN experience, preferably in an acute care, hospice, LTC, or home health setting
  • Ability to travel to patient homes within the service area
  • A valid state-issued driver's license and automobile liability insurance
