Home Care and Hospice Liaison Specialist

Liberty Home Care and Hospice is seeking a skilled Home Care and Hospice Liaison to join our team. As a liaison, you will play a critical role in coordinating the transfer of patients to home health and hospice services, ensuring uninterrupted continued medical care.
Key Responsibilities:
- Coordinate health care institution and physician office activities to facilitate patient transfers.
- Establish relationships with health care institutions to manage patient transfers and coordinate services.
- Collaborate with Branch Management and other intra-company contacts to ensure seamless patient care.
- Assist with recruitment activities to attract qualified staff.
- Develop and maintain knowledge of home health and hospice services, federal, state, and ACHC guidelines.
Requirements:
- Bachelor's degree or LPN, RN, SP, LPT, MSW.
- Two years' experience working with home health, hospice, hospitals, physicians, HMO's, outpatient services, or social services.
- Knowledge of Medicare/Medicaid home health and hospice regulations preferred.
- Ability to communicate effectively with patients, families, and healthcare professionals.
- Strong interpersonal and telephone skills.
