Position Summary
The Clinical Nurse Case Coordinator is responsible for enhancing patient-centered care by managing the patient's care plan, overseeing their length of stay, ensuring efficient resource utilization, and developing a safe discharge plan in collaboration with the healthcare team.
Key Responsibilities:
As a Clinical Nurse Case Coordinator, you will:
- Provide orientation to patients and their families regarding the case management process and available resources.
- Act as the primary educator for patients and families, ensuring their involvement in care discussions.
- Implement treatment plans that align with the patient's individual needs and preferences.
- Document assessments and communicate any social or financial needs of the patient.

Qualifications:
To be considered for this role, you should possess:
- An Associate Degree in Nursing or a Nursing Diploma (Bachelor's Degree preferred).
- A minimum of 2 years of experience in case management or 3 years in clinical nursing.
