Key Responsibilities:
- Conduct thorough initial assessments and develop comprehensive transition plans to meet the unique needs of each patient.
- Participate in multidisciplinary rounds to ensure effective communication and collaboration among healthcare teams.
- Perform utilization reviews to optimize resource allocation and ensure the highest quality of care.
- Provide patient advocacy and support to ensure their needs are met and their voices are heard.
- Delegate tasks to Case Management assistants as needed to ensure efficient workflow.

- Three or more years of clinical experience in a healthcare delivery system, with a strong background in nursing and case management.
- Current Nursing License in the State of MA is required.
