Job Summary:

As a Home Health Physical Therapist, you will play a vital role in providing high-quality patient care in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized treatment plans that promote physical, social, and mental well-being.

Responsibilities:
  • Conduct comprehensive evaluations to assess patient needs and develop personalized treatment plans.
  • Provide evidence-based physical therapy interventions to improve patient function and mobility.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Supervise and mentor physical therapy assistants as needed.
Requirements:
  • Bachelor's Degree in Physical Therapy or equivalent required.
  • Master's Degree in Physical Therapy or related field preferred.
  • Current Physical Therapy licensure in the state of Oregon required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
