Nurse Care Coordinator

TotalMed is seeking a skilled Registered Nurse to join our team as a Step Down Unit Nurse in Providence, Rhode Island.
As a Step Down Unit Nurse, you will be responsible for providing specialized care to patients transitioning from critical care to general medical-surgical units. Your primary goal will be to ensure optimal outcomes for these patients, collaborating with the interdisciplinary team to achieve this objective.
- You will monitor and assess patients' vital signs, cardiac rhythms, and respiratory status in the step-down unit, administering medications and performing procedures as needed.
- You will educate patients and families on post-intensive care recovery, medication management, and self-care techniques, empowering them to take an active role in their healthcare.
- You will communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes in the step-down unit.
The ideal candidate will have at least two years of recent experience in a Step Down setting, with strong communication and adaptability skills. You will also possess a current RN license and BLS certification, with other certifications as required by the facility.
