About the Role

We are seeking a highly skilled HCBS Wait List Care Coordinator to join our team at UnitedHealthcare. As a key member of our care coordination team, you will play a critical role in ensuring that individuals receive the necessary services to achieve their goals and live their best lives.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Identify and initiate referrals for social services, including financial, psychosocial, and community-based programs
  • Communicate effectively with stakeholders to ensure timely and quality care
  • Advocate for individuals and families to ensure their needs and choices are represented and supported

Requirements

To be successful in this role, you will need:

  • Bachelor's Degree in Social Work, Rehabilitation, Nursing, Psychology, Special Education, or related field
  • 1+ years of experience working with individuals with long-term care needs
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams
  • Ability to travel 75% of the time and maintain a valid driver's license
Preferred Qualifications

We also welcome candidates with:

  • Licensed Social Worker or clinical degree
  • Experience working with individuals receiving services on home and community-based waivers in Kansas
  • Background in managing populations with complex medical or behavioral needs
