Overview

Novant Health is seeking a skilled Hospice Liaison Nurse to join our team. As a key member of our hospice care team, you will be responsible for clinically assessing and admitting patients to hospice care, as well as facilitating discharge planning from the acute inpatient setting.

Responsibilities
  • Collaborate with healthcare partners, facilities, patients, and their loved ones to coordinate care and discharge planning.
  • Provide excellent nursing assessment skills and judgment to ensure accurate patient referrals.
  • Stay up-to-date on Medicare/Medicaid rules and regulations to determine patient eligibility for admission.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions.
  • Utilize strong writing skills, organizational, and analytical skills to document patient information and coordinate care.
Requirements
  • Current RN licensure in the appropriate state.
  • Minimum three years' experience in home care or hospice.
  • Previous experience in managed care, infusion care, and case management preferred.
  • Excellent nursing assessment skills, judgment, and understanding of hospice philosophy.
  • Strong writing, organizational, and analytical skills.
  • Ability to interact effectively with diverse individuals at all levels of authority.
