Job Summary

VIA Health Partners is a leading provider of end-of-life care, dedicated to serving patients and families with compassion and expertise. We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in the Charlotte region.

Job Responsibilities

  • Provide high-quality nursing care to patients in their homes or in our facilities, focusing on comfort, dignity, and quality of life.
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings accurately and thoroughly.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices in hospice and palliative care.

Requirements

  • Diploma from a Board-approved nursing program and current, active LPN licensure in the state of North Carolina or South Carolina.
  • Minimum 1 year of experience in direct patient care, preferably in hospice and palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Computer proficiency and ability to navigate electronic medical records.
