Job Summary

Novant Health is seeking a skilled Hospice Liaison Nurse to join our team. As a Hospice Liaison Nurse, you will be responsible for clinically assessing and admitting patients to Hospice care, as well as facilitating discharge planning from the acute inpatient setting. You will also coordinate care and discharge planning with healthcare partners, facilities, patients, and their loved ones.

Responsibilities

As a Hospice Liaison Nurse, your responsibilities will include:
  • Providing clinical assessments and admitting patients to Hospice care
  • Facilitating discharge planning from the acute inpatient setting
  • Coordinating care and discharge planning with healthcare partners, facilities, patients, and their loved ones
  • Collaborating with the interdisciplinary team to develop and implement patient care plans
  • Ensuring that patients receive high-quality, patient-centered care
  • Documenting patient information and progress in a timely and accurate manner
Requirements

To be successful in this role, you will need to have:
  • A current RN licensure in the appropriate state
  • A minimum of three years' experience in home care or hospice
  • Excellent nursing assessment skills and judgment
  • Knowledge of Hospice services and Medicare/Medicaid rules and regulations
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
