Job Summary: We are seeking a Clinical Liaison to serve as an advocate for patients during the pre-admission process. The ideal candidate will work closely with clinical teams to evaluate patients' statuses and medical needs, developing results-producing relationships with hospitals and healthcare professionals.

About the Role: As a Clinical Liaison, you will be responsible for performing clinical assessments, counseling with patient families, and partnering with hospital case managers to create smooth patient transfer processes.

Key Responsibilities:

  • Performing clinical assessments of patients
  • Counseling with patient families to provide support and guidance
  • Partnering with hospital case managers to develop effective patient transfer plans
  • Collaborating with healthcare professionals to ensure seamless care transitions

Requirements:

  • Registered nurse or respiratory therapist with a strong clinical background
  • Problem-solving skills and ability to work autonomously
  • Excellent communication and interpersonal skills
