Job Summary:

The Sacramento Native American Health Center is seeking a highly skilled and compassionate Behavioral Health Coordinator to join our team. As a key member of our Behavioral Health department, you will play a vital role in providing exceptional care to our patients and contributing to the growth and transformation of our organization.

Key Responsibilities:

Provide brief, problem-focused interventions to address primary mental health concerns and support behavioral changes to improve outcomes.
Perform risk assessment and crisis management, including evaluation and referral to Emergency Services for patients who are suicidal, homicidal, or experiencing intimate partner violence.
Document all patient interactions in the Electronic Health Record according to agency procedures and timelines.
Make referrals to specialty mental health and other specialty services within the agency as appropriate.
Assist patients with tasks associated with community functioning, such as filling out paperwork for public benefits, referral to higher level of care and supportive programming, vocational rehab, social support, housing, food, and clothing closets, etc.

Requirements:

Master's degree or higher in a related field.
Experience with brief interventions, such as Motivational Interviewing.
Training in de-escalation techniques.
Excellent computer skills, preferably with Windows, including Microsoft Office Suite.
Excellent telephone and communication skills.
Ability to work independently, set priorities, and work well under pressure.
Ability to maintain a high degree of confidentiality.
Ability to demonstrate superior professionalism when dealing with patients, subordinates, colleagues, community members, and vendors.

Preferred Qualifications:

Master's in social work preferred.
1+ year working in mental health.
Knowledge of common issues facing the clinic population.
Experience with Nextgen or other electronic health record system.
Knowledge of Third Party and/or FQHC billing.
Experience in an FQHC setting.
Knowledge of traditional, cultural, and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.
