Job Summary

We are seeking a highly skilled Behavioral Health Clinician II to join our team at One Community Health. As a key member of our multidisciplinary team, you will provide counseling services to individual patients, their families, and significant others, as well as group sessions.

Key Responsibilities
  • Conduct individual and group therapy sessions using strength-based approaches.
  • Develop personalized treatment plans in collaboration with patients, setting goals, objectives, and discharge plans.
  • Collaborate with physicians, nurses, case managers, and other therapists to address patient needs and coordinate care.
  • Make referrals for patients requiring additional psychological or psychiatric services, including psychotropic medication if necessary.
  • Participate in integrated systems of care, staff meetings, case reviews, and consultation/supervision sessions as required.
Requirements
  • Master's degree or higher in Social Work.
  • LCSW State license or ACSW in good standing required.
  • Experience providing therapy and counseling services, preferably in a healthcare or mental health setting.
  • Strong interpersonal skills and the ability to build rapport with patients from diverse backgrounds.
  • Excellent communication skills, both verbal and written.
  • Ability to work effectively in a multidisciplinary team environment.
Preferred Background
  • Experience in areas such as Substance Abuse Disorder, Maternal Mental Health, Pediatrics, or LGBTQIA+ and Gender Affirming Health.
  • Bilingual skills, with English plus any of the following languages: Spanish, Dari, or Pashto.
