Service Coordinator - Registered Nurse

University Health System- San Antonio seeks a skilled Service Coordinator - Registered Nurse to provide exceptional care and support to our STAR Kids Members.

Key Responsibilities:
  • Conduct comprehensive assessments of Members' needs and preferences, providing education and health-related information to ensure optimal outcomes.
  • Coordinate and manage Members' covered and non-covered services, including acute and long-term care, to meet their physical, behavioral, and psychosocial needs.
  • Develop and implement personalized care plans, collaborating with healthcare professionals and Support Networks to ensure seamless care transitions.
  • Maintain accurate and up-to-date records, adhering to University Health System- San Antonio's policies and procedures.
Requirements:
  • BSN degree or equivalent required; four years of clinical experience preferred.
  • Working knowledge of ICD10, CPT4 coding, InterQual/M&R criteria, Texas Department of Insurance HMO standards, LTSS, and NCQA standards preferred.
  • Current licensure with the Texas State Board of Nurse Examiners and certification from a recognized professional agency preferred.
Licensure and Certification:
  • RN licensure with the Texas State Board of Nurse Examiners required.
  • Current certification from a recognized professional agency preferred.
