Key Responsibilities:

  • Perform detailed evaluations and deliver continuous primary healthcare to participants
  • Create personalized care strategies in collaboration with physicians and the healthcare team
  • Support discharge planning and ongoing management of care
  • Work together to ensure efficient care coordination and resource utilization
  • Analyze data for ongoing enhancement of care methodologies
  • Participate in after-hours and weekend on-call duties

Required Qualifications:

  • Degree in nursing from an accredited institution
  • Certification as a Nurse Practitioner with authority to prescribe medications
  • Active registered nursing state license
  • At least two years of experience working with the elderly population
  • Familiarity with geriatric healthcare and chronic illness management
  • Valid driver's license and vehicle insurance
