Job Title: Nurse Practitioner - Primary Care Provider

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team as a Primary Care Provider. As a key member of our interdisciplinary team, you will work collaboratively with physicians and other healthcare professionals to provide high-quality, patient-centered care to our seniors.

Job Summary:

The Primary Care Provider will be responsible for performing comprehensive assessments, developing care plans, and providing ongoing medical care to our participants. You will work closely with our team to ensure seamless care coordination and effective communication with participants, families, and healthcare providers.

Key Responsibilities:
  • Perform comprehensive assessments, including initial, semi-annual, and unscheduled assessments, to identify participants' medical needs and develop individualized care plans.
  • Collaborate with physicians and other healthcare professionals to develop and implement care plans that meet participants' unique needs.
  • Provide ongoing medical care, including medication management, wound care, and other medical interventions, as needed.
  • Work closely with our team to ensure seamless care coordination and effective communication with participants, families, and healthcare providers.
  • Participate in rotating night and weekend call to ensure 24/7 coverage and respond to urgent care needs.
Requirements:
  • Graduation from an accredited school of nursing.
  • Certificate as a Nurse Practitioner with prescriptive authority.
  • NP state license.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care and geriatric issues and best practices.
