Job Title: Nurse Practitioner

At Salgado LLC, we are seeking a highly skilled and compassionate Nurse Practitioner to join our team in our Outpatient Clinic. As a key member of our healthcare team, you will provide high-quality patient care, working closely with our Medical Director and other healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities:

  • Conduct comprehensive health assessments, develop and implement individualized care plans, and provide education to patients and their families.
  • Order and interpret lab tests, perform procedures, and document patient care in our electronic medical record system.
  • Manage medication orders, coordinate patient care, and refer patients to specialists as needed.
  • Participate in interdisciplinary team meetings, collaborate with healthcare professionals, and provide education and support to staff, patients, and families.
  • Document patient changes, communicate with team members, and evaluate patient progress.
  • Precept medical students and residents during their clinic rotation, and participate in quality improvement initiatives.

Qualifications:

  • Master's degree in clinical nursing from an accredited Nurse Practitioner program.
  • Active NP license, 2-5 years of nursing experience, and experience working with a frail or elderly population preferred.
  • Reliable transportation, a valid driver's license, and state-required liability auto insurance.
  • Medically cleared for communicable diseases and up-to-date immunizations before engaging in direct patient contact.
  • Pass a comprehensive criminal background check, including federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
