This is a 4-day work week position with a 10-hour shift, offering a unique opportunity to balance work and personal life. As a Nurse Practitioner, you will be responsible for performing in-person comprehensive assessments, collaborating with physicians and IDT to develop care plans, and maintaining the care of participants in the nursing home.
We are looking for a candidate with a Master's Degree from an accredited school of nursing, a Certificate as a Nurse Practitioner with Prescriptive Authority, and a minimum of two years of experience working with the senior population. You must be fully vaccinated at the time of hire and have a current NP license to practice medicine in California.
Essential Job Duties:
- Perform in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, and continuous medical coverage to participants.
- Collaborate with physicians and IDT to develop care plans for each participant.
- Collaborate with physicians and IDT in discharge planning from acute or long-term settings.
- Help maintain care of participants in the nursing home, making regular visits as dictated by participant need.
- Work collaboratively with our physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination.
- Review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements.
- Participate in rotating night and weekend call.
Requirements:
- Master's Degree from an accredited school of nursing.
- Certificate as a Nurse Practitioner with Prescriptive Authority.
- RN State License.
- Minimum of two years of experience working with the senior population.
- Current knowledge of chronic care/geriatric issues and best practices.
- Must be fully vaccinated at the time of hire.
- Must have a current NP license to practice medicine in California.
Skills and Certifications:
- Certificate as a Nurse Practitioner with Prescriptive Authority.
- CA Licensed.
- Master's Degree.
