Case Planner

We are seeking a highly motivated and dedicated Case Planner to join our team at Children's Aid. As a Case Planner, you will play a critical role in supporting families and children in need by providing case management services, conducting assessments, and developing intervention plans.
Job Summary:The Case Planner will work closely with families, social workers, and other stakeholders to identify and address the needs of children and families. This includes conducting home visits, providing support and guidance to families, and connecting them with community resources.
Key Responsibilities:- Conduct home visits and assessments to identify the needs of children and families
- Develop and implement intervention plans to address the needs of children and families
- Provide support and guidance to families to help them access community resources and services
- Collaborate with social workers and other stakeholders to ensure a comprehensive approach to supporting families
- Maintain accurate and up-to-date records of case management activities
- Bachelor's degree in Social Work or a related field
- Experience working with families and children in a case management or social work setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work a varied schedule, including evenings and weekends
