Social Work Lead

As a Social Work Lead at Bryan Health, you will play a vital role in directing the discharge planners within the Care Management team. Your primary responsibility will be to plan successful discharge plans for patients and families, focusing on setting up patients and their families for safe and successful outcomes and decreasing re-admissions. You will assist clients by assessing their situation, setting goals, and obtaining required services. Additionally, you will determine the nature of specific client situations by assessing medical, psychological, emotional, and social information. You will work closely with the Care Management team and other members of the interdisciplinary team to ensure seamless coordination of care.
Key Responsibilities:
1. *Commit to the mission, vision, beliefs, and consistently demonstrate our core values. 2. *Maintain regulatory requirements, including all state, federal, and accreditation regulations. 3. *Support patients in making informed decisions by acting as their advocate regarding patient clinical status and treatment options. 4. *Perform a psychosocial assessment and reassessment on all patients, addressing motivational and psychosocial issues. 5. Maintain open communication interdepartmentally and interdisciplinary. 6. Ensure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels. 7. Handle case assignments, review concurrent case progress, and adjust plans accordingly. 8. Record and document case information, accurately completing all necessary forms and producing statistical reports. 9. Interact professionally with patient/family and involve them in forming the plan of care. 10. Gain knowledge of patient's financial status and diagnosis to better assist with patient's discharge needs. 11. Prepare patients' discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements, orienting and training family members, and providing resources. 12. Perform and evaluate mental health, drug, and ETOH assessments. 13. Provide immediate counseling/support of patients and family, effectively diffusing crisis and referring to appropriate services for abuse/neglect as needed. 14. Facilitate ongoing communication with Child Protective Services (CPS), law enforcement, and county attorney's office. 15. Under provisional supervision, provide crisis/supportive counseling and coaching to patients, including a treatment plan for short-term and long-term goals for the patient. 16. Provide supervision and education to bachelor-level social workers within the case management department. 17. Provide supervision to bachelor-level social work students completing practicum. 18. Provide education to patients and providers regarding insurance coverage, including Medicaid eligibility. 19. Facilitate patient engagement and follow-up care by providing education and support to patients/families regarding chronic conditions. 20. Strengthen community relationships. 21. Track and trend delay in Care Coordination and present results to Utilization Review Committee. 22. Assist with scheduling the Care Coordination department. 23. Lead and assist with problem-solving within the Care Coordination department, outside facilities, and the interdisciplinary team. 24. Maintain professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in the field of expertise. 25. Participate in meetings, committees, and department projects as assigned. 26. Perform other related projects and duties as assigned.
Requirements:
1. Maintain clinical competency as required for the unit, including age-specific competencies relative to patient's growth and developmental needs, annual skill competency verification, and mandatory education and competencies. 2. Knowledge of management and leadership principles and practices. 3. Knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage. 4. Knowledge of evidence-based counseling techniques to the degree necessary to provide appropriate emotional support to patients and their families. 5. Knowledge of computer hardware equipment and software applications relevant to work functions. 6. Ability to submit and interpret Pre-admission Screening and Resident Review (PASRR). 7. Ability to communicate effectively both verbally and in writing. 8. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff. 9. Ability to problem-solve and engage independent critical thinking skills. 10. Ability to deal with resistance and complex situations. 11. Ability to maintain confidentiality relevant to sensitive information. 12. Ability to prioritize work demands and work with minimal supervision. 13. Ability to maintain regular and punctual attendance.
Education and Experience:
Bachelor's degree in social work required. Current license as a Social Worker in the State of Nebraska required. Master's degree in social work from accredited university preferred. Five (5) years of medical social work experience preferred.
