Job Summary

The Clinical Care Manager is a critical role within our organization, responsible for ensuring that our members receive timely access to quality, medically necessary behavioral health services. As a Clinical Care Manager, you will be the primary point of contact for members, providers, and other stakeholders, working collaboratively to develop and implement individualized care plans that address the unique needs of each member.

Key Responsibilities
  • Conduct utilization reviews to ensure that members receive medically necessary services
  • Develop and implement care plans that address the unique needs of each member
  • Collaborate with providers, members, and other stakeholders to ensure seamless care coordination
  • Monitor member outcomes and make adjustments to care plans as needed
  • Document all interactions and care plan developments in our electronic system
Requirements
  • Master's degree in Social Work or related field
  • License eligible and willing/able to obtain licensure within the first year of employment
  • Minimum of 3 years of direct clinical care experience working with adults, children, adolescents, and/or their families in behavioral health settings
  • Excellent clinical case conceptualization skills, meeting facilitation, proactive planning, customer service, and verbal and written communication skills
Work Environment

The Clinical Care Manager will work in a fast-paced, dynamic environment, collaborating with a multidisciplinary team to ensure that our members receive the highest quality care. The successful candidate must be able to travel within Philadelphia and surrounding counties, and must be able to adapt to continuous changes/demands of the job.
