Job Description

This exciting role involves supporting patients in our Hospital at Home program through routine and urgent in-home visits, telephone triage, and coordination of care with our team and community partners. You will be responsible for delegating and supervising others, working all shifts, including weekends and holidays, and standing for prolonged periods.
