We are currently looking for a dedicated Practical Nursing Specialist to join our ICM/ACT team.


This role offers a range of benefits that you may find appealing, including:

  • A Monday to Friday schedule with daytime hours, totaling 40 hours per week, in both office and community settings, with a company vehicle provided for travel.


The Practical Nursing Specialist will be an integral part of the ICM/ACT multidisciplinary team, focusing on the coordination of both behavioral and physical health care for program participants. Responsibilities include monitoring medication adherence and delivering psychiatric nursing services in alignment with physician directives.


Key Responsibilities:

  • Collaborate with participants and team members to develop, implement, evaluate, and adjust recovery plans.
  • Manage a caseload of assigned participants, ensuring timely completion of necessary documentation.
  • Serve as a liaison, engaging with community organizations and families to maintain cohesive care throughout the treatment process.
  • Conduct ongoing assessments of participants' mental health symptoms and adjust recovery plans accordingly.

Qualifications:
  • Must hold a Board Certified LPN license in Delaware or be part of the Nursing Licensure Compact.
  • Familiarity with mental health and substance abuse support systems.
  • Strong verbal and written communication skills.
  • Ability to work autonomously.
  • Valid driver's license with a satisfactory driving record.
  • Excellent organizational abilities.

Work Environment:

  • Frequent communication and active listening are essential.
  • Physical requirements include lifting, carrying, kneeling, and bending, which may involve direct participant interaction.
  • Local travel is necessary, including the transportation of participants.
  • Availability to work during emergencies, including adverse weather conditions, may be required.
  • Crisis intervention skills may be needed in urgent situations.
  • Flexibility in scheduling and potential on-call duties are expected.
