Our Team:
We are seeking a talented Licensed Practical Nurse (LPN) to join our team as a Home Health Specialist. As a key member of our team, you will work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients.
Responsibilities:
Assess, plan, direct, and evaluate each patient's care plan to ensure it meets their needs. Conduct patient home visits, manage emergency situations, and provide nursing care in accordance with applicable regulations and Company policy. Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
Qualifications:
Active LPN state license, valid driver's license, and two or more years of applicable nursing experience. We offer a competitive salary, flexible hours, and a collaborative work environment.
