Transforming Healthcare Delivery

Aledade is seeking a highly skilled Practice Transformation Specialist to join our team. As a key member of our organization, you will play a critical role in empowering independent primary care practices to deliver better care to their patients and thrive in value-based care.

As a Practice Transformation Specialist, you will work closely with our practices to help them design, deploy, and train staff on workflows to incorporate our technology into their daily activities. You will be responsible for project management, data analysis, and problem-solving to ensure efficient and effective progression of initiatives.

We are looking for a candidate with a solid foundational understanding of the healthcare delivery system, clinical and administrative experience, and exceptional change management skills. If you are passionate about population health and highly mission-oriented around providing better care to patients while reducing costs, we encourage you to apply.

Key Responsibilities:
  • Project management and coordination
  • Data analysis and reporting
  • Problem-solving and process improvement
  • Partnership with Medical Director and Executive Director on project-specific timelines
  • Communication and stakeholder management
  • Best practice sharing and collaboration with teammates
  • Proposal and report preparation
Requirements:
  • Residency in PA, NJ, DE, or southern NY and willingness to travel up to 20% in your region
  • Graduate of an accredited college or university in nursing, social work, health technology, or information technology-related programs
  • 4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted
  • 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
