Position Summary:
The Licensed Practical Nurse (LPN) is responsible for delivering professional nursing care to residents of varying ages in long-term care settings, under the guidance of a registered nurse, advanced practice provider, or physician. The LPN will demonstrate a comprehensive understanding of nursing principles relevant to the clinical environment.
The role includes:
- Exhibiting competency within the full scope of nursing expertise, adhering to age and population-specific standards related to residents' physical and psychosocial needs.
- Conducting assessments and administering medications or treatments as prescribed.
- Providing technical support to healthcare professionals as necessary.
- Documenting resident interactions and outcomes related to care, medication, behavior, activity, and dietary needs.
- Building and maintaining effective working relationships with residents, healthcare providers, and the community.
- Communicating clearly while ensuring confidentiality of sensitive information.
- Counseling residents and family members on prevention and treatment plans under the direction of a registered nurse or physician.
Additional responsibilities may include performing other related duties as assigned.
Qualifications:
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse. An Associate degree in practical nursing is preferred.
- Must hold an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing Board and/or possess multi-state licensure privileges.
- Completion of required annual continuing education units (CEU) and competency training requirements of the department and facility.
