Job Summary

We are seeking a highly skilled Patient Care Coordinator to join our team at Providence Health & Services. As a key member of our care delivery team, you will play a vital role in ensuring seamless care delivery for patients in a surgical or non-surgical setting.

Key Responsibilities
  • Act as a patient advocate to ensure access to healthcare during the patient's entry into and through the services of the clinic.
  • Utilize communication and management skills to coordinate care delivery and ensure a high level of customer service.
  • Work under the general direction of a physician or designee to provide support and guidance as needed.
  • Collaborate with the care team to ensure a comprehensive and coordinated approach to patient care.
Requirements
  • 1 year of experience in a medical or healthcare setting, or 2 years in a customer service role.
  • Prior experience providing a high level of customer service in a fast-paced environment, including handling confidential data with discretion.
Preferred Qualifications
  • Electronic Medical Records (EMR) experience.
