At True Care NY, we are seeking a dedicated and compassionate Patient Advocate to join our team. As a Patient Advocate - NHTD/TBI Waiver Program, you will play a vital role in guiding prospective participants through the enrollment and onboarding processes for the NHTD/TBI Waiver program. Your primary responsibility will be to provide comprehensive support and advocacy for individuals with disabilities and their families.
Key Responsibilities:- Act as an advocate for prospective participants, providing guidance throughout the waiver program approval process.
- Collaborate with internal departments to ensure seamless care coordination and timely interventions.
- Facilitate access to community resources and other forms of support, promoting independence and quality of life.
- Maintain accurate and comprehensive documentation, meeting all timeliness requirements.
- Bachelor's Degree or higher, with at least two years of experience in case management, service coordination, or related fields.
- General knowledge of developmental, physical, and cognitive disabilities, traumatic brain injury, chronic medical conditions, and mental illness.
- Fluency in Spanish (required) and proficiency in Microsoft Suite and EMR systems.
- Strong verbal and written communication skills, with empathy and professionalism.
