Position Overview:
Brain Injury Services (BIS) is dedicated to assisting individuals with brain injuries in achieving their personal goals and enhancing their quality of life. We are currently seeking a full-time Veteran Support Case Manager to work with our veteran clientele. Ideal candidates will possess a military background and have experience managing both general and veteran-specific caseloads. Preference will be given to those familiar with Medicaid processes, including referrals, service authorizations, billing oversight, and collaboration with Managed Care Organizations (MCO) in the Targeted Case Management (TCM) framework.
Role Definition:
Under the guidance of the Department Manager for Adult Case Management Services, the Veteran Support Case Manager will provide community-oriented case management for adults with acquired brain injuries and their families in the Northern Virginia area. This role encompasses long-term case management and public awareness initiatives, delivered through a community-based approach.
Key Responsibilities:
- Uphold the agency's integrity by adhering to professional standards and regulatory requirements.
- Act as the primary contact for all veteran-related referrals and consultations.
- Gather information and facilitate referrals to appropriate services, including those specific to veterans and TCM.
- Conduct comprehensive intake evaluations and maintain accurate documentation as outlined in the Policy and Procedure Manual.
- Complete all necessary paperwork related to Medicaid TCM services, including authorizations, assessments, and billing.
- Perform thorough client assessments to identify strengths, weaknesses, and support systems within 45 days of case initiation.
- Ensure compliance with confidentiality laws and maintain appropriate documentation.
- Foster strong therapeutic relationships with clients and their families.
- Assist clients in developing personalized goals and plans based on their needs assessments.
- Review Individualized Service Plans (ISPs) regularly and ensure client participation in service delivery.
- Coordinate and monitor community services to align with client goals.
- Maintain regular contact with clients and families to provide support and guidance.
- Advocate for clients within their communities to secure necessary services.
- Travel to clients' homes and communities to provide services.
- Ensure a professional home environment for remote work, with reliable internet access.
- Manage a caseload of up to 30 active clients and no more than 15 follow-along clients.
- Provide services to non-veteran clients as directed.
- Understand and prioritize the Vendor Contract budget based on client needs and resource availability.
- Educate clients and the community about brain injuries and available resources.
- Commit to person-centered service delivery, allowing clients to choose their services and providers.
- Maintain a flexible schedule, including evening meetings as necessary.
- Engage in ongoing professional development and training.
- Participate in departmental meetings and supervision sessions.
- Perform additional duties as assigned by the Department Manager.
Qualifications:

• Ability to work independently with strong problem-solving skills.

Essential Requirements:
• Valid driver's license and reliable transportation for local travel.

• Access to secure internet services for remote work.
• Proficiency in reading, writing, and proofreading at a professional level.
Educational Background:
• Master's Degree in a relevant human services field (e.g., Social Work, Rehabilitation Counseling, Psychology, Occupational Therapy).
• Completion of Brain Injury Fundamental Training and Qualified Brain Injury Specialist training upon hiring.
• Optional Certified Brain Injury Specialist training after one year of employment.
