About the Role:
We are seeking a skilled and compassionate MDS Coordinator to join our Clinical Services team at Lifespace Communities. As an MDS Coordinator, you will play a vital role in ensuring the highest quality of care for our residents by coordinating and documenting their care plans.
Key Responsibilities:
- Participate in interdisciplinary team meetings to review and plan resident care.
- Accurately and timely document resident conditions and nursing needs.
- Report pertinent observations and reactions regarding residents in a timely manner.
- Oversee and provide leadership to licensed practical nurses and certified nursing aides.
- Develop, direct, and monitor nursing assistant assignments based on census and level of care required.
- Execute treatments as necessary while documenting status and observing reactions to medications and treatments.
- Initiate physician orders, verify accuracy, and administer medications and treatments according to orders.
- Facilitate communication with families regarding changes in medications and/or changes in the resident.
- Establish protocols to ensure care plans are continually reviewed for updated and accurate data.
Requirements:
- Nursing diploma or associate degree in nursing from an accredited nursing program.
- One-year experience working in a long-term care facility.
- Certifications and Registered Nurse license and other licensure required by state regulations.
