This role involves extensive travel across various regions.
ROLE SUMMARYThe Home Infusion Therapy Nurse utilizes clinical expertise to deliver high-quality infusion therapy within the comfort and safety of patients' residences.
KEY RESPONSIBILITIES- Gathering comprehensive medical histories from patients and their families.
- Ensuring adherence to established policies, protocols, and best practices.
- Conducting thorough physical assessments, including vital signs and mental status evaluations.
- Creating and executing individualized nursing care plans in collaboration with the supervising physician.
- Fostering positive relationships with team members and external service providers to ensure seamless continuity of care.
- Completing and submitting patient visit documentation in accordance with policy.
- Monitoring the effectiveness of medication regimens, developing care plans, and providing education to patients and their families.
- Assessing the home environment to identify available support from family members or caregivers.
- Observing and reporting any signs, symptoms, or reactions to treatments to the physician.
- Coordinating patient care to achieve optimal health outcomes.
- Traveling as required to deliver home infusion therapy and being available to patients around the clock.
- Adhering to safety protocols to prevent injury and safeguard equipment.
- Consulting with the Clinical Director regarding potential modifications to treatment plans.
- Participating in annual continuing education related to infusion therapy.
- Performing additional duties as assigned.
- Exceptional oral and written communication skills.
- Proficient understanding of medical terminology for effective communication with healthcare professionals and patients.
- Strong attention to detail.
- Ability to prioritize and manage multiple responsibilities independently.
- Basic computer skills and typing proficiency.
- Patient-centered approach.
This position requires a valid Registered Nurse License in the relevant state, completion of home infusion training, and a minimum of one year of experience in a home health setting. Current certifications in BLS and ACLS are also necessary.
