Job Title: Patient Care Coordinator

Community Health Network is seeking a skilled Patient Care Coordinator to join our team in the Cardiac Progressive Care Unit. As a key member of our nursing staff, you will provide daily leadership and support to ensure the highest level of patient care and satisfaction.

Responsibilities:
  • Provide shift-to-shift leadership and support to the nursing team
  • Exhibit excellence in clinical patient care, customer service, critical thinking, schedule management, relationship competencies, and organizational skills
  • Work collaboratively with a great team in a fast-paced environment
  • Care for a variety of patients, including cardiac, surgical, and medical cases
  • Develop and implement effective care plans to meet patient needs
  • Collaborate with interdisciplinary teams to ensure seamless patient care
Requirements:
  • Graduate of a National League for Nursing (NLN) accredited school of nursing
  • Three years of related professional nursing experience
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana
  • 2+ years experience as an RN within specialty area or related specialty area
