Acute Care Discharge Planning Specialist

Responsibilities:
- Conduct thorough assessments of patients' medical needs and develop personalized plans of care.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments.
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and adherence to treatment plans.
Requirements:
- Current RN licensure in the state of practice
- 2+ years of experience in acute hospital settings, preferably as a case manager or discharge planner
- BLS certification
Salary Range: $130,000 - $160,000 per year
