Post-Acute Care Navigator

At Enhabit Home Health & Hospice, we're committed to expanding what's possible for patient care in the home.
We're seeking a skilled Post-Acute Care Navigator to join our team. As a key member of our team, you'll assist patients in navigating post-acute care, creating positive impacts on patient outcomes and referral source satisfaction.
This role involves integrating evidence-based clinical guidelines, preventative guidelines, protocols, and metrics into patient-centered transition plans that promote quality and efficiency in post-acute care delivery.
You'll represent our area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups, as well as executive-level opportunities.
Key Responsibilities- Assist patients in navigating post-acute care
- Develop patient-centered transition plans that promote quality and efficiency
- Represent our area branches in strategic relationships with healthcare organizations
- Graduate degree from an approved school of nursing, therapy, or social work
- Licensed in the state where they practice
- Two years' demonstrated field experience
- Demonstrated experience with federal, state, and local laws governing home health and hospice operations
- Basic technology skills, including mobile device operation
