Job Summary

We are seeking a highly skilled Community Liaison Worker to join our team at NYC Health + Hospitals/Metropolitan. The successful candidate will be responsible for coordinating psychiatric out-patient appointments, assisting patients with referrals for entitlements and community resources, and tracking patient data in our Psychiatric Discharge Management System.

Key Responsibilities
  • Coordinate psychiatric out-patient appointments for patients residing outside of the hospital's catchment area
  • Assist patients with referrals for entitlements and community resources
  • Track patient data in the Psychiatric Discharge Management System
  • Meet with patients prior to discharge to explain the Koskinas Program and obtain agreement for outreach services
  • Call patients, community agencies, and treatment resources within 72 hours of discharge to determine successful compliance with discharge plans
Requirements
  • One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services
  • Completion of an approved six-month training program in community liaison work, plus six months of full-time experience
