Home Health Nurse

We are seeking a compassionate and skilled Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Nurse, you will be responsible for delivering personalized care that improves health outcomes and enhances the overall well-being of our patients.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting vital signs assessments.
- Develop and implement individualized care plans in collaboration with patients, their families, and other healthcare professionals.
- Collaborate with primary care physicians and other healthcare providers to ensure seamless care transitions.
- Conduct discharge planning to ensure patients are adequately prepared for hospital discharge or transition to long-term care facilities.
- Utilize medical records to identify patient needs and develop targeted treatment plans.
- Administer injections and perform catheter care as needed.
- Provide critical care to patients with complex medical conditions.
- Utilize internal medicine knowledge to diagnose and treat patients with chronic health conditions.
- Monitor patient progress and adjust treatment plans as necessary.
Requirements:
- Proven experience as a Registered Nurse with a strong background in home health care.
- Experience administering injections.
- Knowledge of family planning and health coaching principles.
- Strong critical care skills, with the ability to assess and manage complex patient conditions.
- Proficiency in medical records management and documentation.
- Ability to take and record vital signs accurately and efficiently.
- Experience with catheter care and maintenance.
- Familiarity with internal medicine and the ability to apply this knowledge in a home health setting.
