Patient Care Associate Job Description

At NYC Health + Hospitals, we are seeking a highly skilled and compassionate Patient Care Associate to join our team. As a Patient Care Associate, you will play a vital role in providing exceptional patient care and support to our patients and families.

Key Responsibilities:
  • Provide physical care to patients, including vital signs, weight, height, blood pressure, and oral intake and output.
  • Prepare patients for examination and/or special procedures and accompany patients as needed.
  • Overserve patients' condition, mental state, and social behavior and report observations to the Nurse-in-Charge.
  • Prepare patients for meal and assist with feedings as needed.
  • Assist and accompany patients during the admission, discharge, or transfer process, ensuring comfort and safety.
  • Turn and position patients as instructed by the RN or designee.
  • Monitor patients on fall precaution and complete the Flow sheet as per policy.
  • Monitor patients on suicidal precaution and notify the nurse of observations and patient behavior.
  • Collect routine specimens, such as urine, stool, sputum, etc.
  • Provide post-mortem care.
  • Inform patients on proper preparation for various medical testing.
  • Prepare patients for examination and assist medical providers during examination by providing necessary supplies/equipment.
  • Perform 12-lead EKG and document appropriately in MISYS.
  • Check vital signs, weight, height as indicated, and document findings on the medical record.
  • Perform screening for blood glucose levels.
  • Perform phlebotomy, print labels, and label specimens accurately.
  • Instruct patients on how to obtain specimens for laboratory testing and perform basic specimen screening/testing through dipstick hemocult.
  • Conduct interviews of clients to obtain routine personal and demographic information.
  • Maintain inventory of the unit's equipment stock and supplies.
Requirements:
  • High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization.
  • Successful completion of a NYS Department of Health prescribed training program for Phlebotomy, satisfactory to the Laboratory Director.
  • Completion of a healthcare setting-based training program in the performance of electrocardiographs and basic life support procedures, including certification in Cardio-Pulmonary Resuscitation.
