Bachelor's degree in a Human Services related discipline or minimum four years of direct case management experience may be substituted for degree requirements. Strong knowledge of community resources, particularly housing for low-income individuals, available in the Clark County service area is essential. Excellent knowledge of and ability to use current Microsoft software computer programs, including Word, Excel, PowerPoint, and Publisher, as well as e-mail, Adobe Acrobat, and the internet, is required.

The selected candidate will provide case management services, including but not limited to, providing appropriate resources, tools, and counseling to assist participants in achieving their case plan goals. They will make referrals for services, follow up to ensure that referrals were completed, and track and document participant progress. This position will conduct comprehensive assessments and develop individualized housing and service plans for each participant using the assessment.

**Salary:** $55,000 - $65,000 per year, depending on experience.

**Responsibilities:

  • Provide case management services to participants, including but not limited to, providing appropriate resources, tools, and counseling to assist participants in achieving their case plan goals.
  • Make referrals for services and follow up to ensure that referrals were completed.
  • Track and document participant progress.
  • Conduct comprehensive assessments and develop individualized housing and service plans for each participant using the assessment.
  • Collaborate with other departments and agency programs for client housing needs.

**Requirements:

  • Ability to maintain a self-motivated and self-directed schedule, address multiple projects in a timely manner, and take projects from beginning through completion.
  • Ability to work with people of diverse backgrounds and circumstances.
  • Knowledge of the Homeless Management Information System (HMIS).

**Physical Requirements:

While performing the duties of this job, the employee is regularly required to walk, stand, climb, balance, stoop, kneel, crouch, crawl, and reach with hands and arms on a continuous basis. The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and ability to focus. The employee must have the ability to operate a telephone, desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
