We are seeking a highly skilled and experienced RN Case Manager to serve as a Clinical Lead in our Case Management department. Key Responsibilities- Assess the needs of the department and collaborate with healthcare team members to coordinate and complete timely assessment, planning, implementation, and evaluation of discharge plans and care transitions.
- Support RN Case Managers in the department according to policies and procedures.
- Assist with daily operational aspects of the department, including facilitation of work priorities.
- Maintain professional educational development and guide the professional growth and development of RN Case Managers.
- Participate in quality improvement activities and regularly perceive potential problem situations to intervene and offset adverse impact.
- Ensure departmental compliance with regulatory standards and act as a resource to staff.
- Identify and communicate special staffing needs and/or problems to the Clinical Manager/Director.
- Massachusetts RN licensure.
- 3 years of RN experience with at least 1 year as a charge/lead RN.
- Current Basic Life Support (BLS) certification.
- Bachelor of Science in Nursing (BSN). MSN preferred.
