Primary Care Nurse

We are seeking a skilled Primary Care Nurse to join our team at Lynn Community Health Center. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and promoting health education to patients and families.
Key Responsibilities- Triage patients by telephone or in person to determine urgency and level of care needed, ensuring appropriate care disposition.
- Perform patient procedures, assess patient health status, and provide health education to patients/families regarding pertinent health issues, clinical conditions, treatments, recommended follow-up actions, and/or home care.
- Communicate pertinent abnormal test results to providers.
- Track patients for necessary follow-up, documenting appropriately.
- Utilize your skills to interact with patients/clients, team members, and staff from other departments.
- Graduate of an accredited nursing program, current Mass. licensure, BLS certification.
- Bilingual in Spanish/Khmer/Russian with knowledge of medical terminology preferred.
- Previous Community Health or Ambulatory Experience preferred.
- A desire to work as a collaborative member of a multi-disciplinary team.
