Telephonic Nurse Case Manager II

Job Summary:
Elevance Health is seeking a highly skilled and compassionate Telephonic Nurse Case Manager II to join our team. As a Telephonic Nurse Case Manager II, you will be responsible for providing high-quality care management services to our members, ensuring they receive the best possible care and support.
Key Responsibilities:
- Conduct thorough assessments of members' health needs and develop personalized care plans to address their objectives and goals.
- Implement care plans by facilitating authorizations and referrals as needed, ensuring members have access to the services they require.
- Coordinate internal and external resources to meet members' identified needs, ensuring seamless care transitions and optimal outcomes.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure members receive the best possible care.
- Collaborate with Medical Directors and Physician Advisors to develop care management treatment plans, ensuring members receive comprehensive and coordinated care.
Requirements:
- Current, unrestricted RN license in applicable state required.
- Minimum of 5 years of clinical experience, with a focus on case management and care coordination.
- Strong communication and interpersonal skills, with the ability to work effectively with members, healthcare providers, and other stakeholders.
- Ability to work in a fast-paced environment, with a high level of accuracy and attention to detail.
Preferred Qualifications:
- Certification as a Case Manager (CM) or Certified Professional in Healthcare Management (CPHM) preferred.
- Experience working in a managed care setting, with a focus on care coordination and case management.
